Our Health Connections Team offers free support to help you improve your health and wellbeing.

  • We help you set goals and make changes that feel right for you.

How we can help

Drop-in support at the practice (and weekly weigh in)

You can visit us in the Live Well Room at the practice for free support. No booking needed. 

At our weekly weigh-in, we can also check your weight and blood pressure to help you track your health goals. 

Drop-in times: 

  • Monday: 10am to 11am 
  • Tuesday: 2pm to 3pm (weekly weigh-in) 
  • Thursday: 9:30am to 10:30am 
  • Friday: 2pm to 3pm 

1 to 1 support

We offer up to 6 sessions with a Health Connector. You have a choice of in person meetings or phone calls

During this support we can: 

  • Listen to what matters to you.
  • Support you to make changes that are important for your wellbeing.
  • Help you make a plan and identify any barriers that may occur in achieving this
  • Connect you to free or low-cost local services to achieve your goals, such as exercise classes and support groups. 

Group Education Programmes

We offer three different group programmes:

  • Live Well With Pain - Learn strategies to manage long-term pain, improve daily life, and feel more in control. 

  • Habits for Health - Build healthier habits around food, activity, sleep, and wellbeing.

  • Share experiences with others in a supportive environment. 
  • Learn practical tips from trained facilitators. 
  • Gain confidence to make changes that matter to you. 

Peer support groups

Our peer support groups give you a safe, welcoming space to meet others who understand what you’re going through.

Sharing experiences can help you feel more connected and give you practical ideas for managing your health. 

Groups we offer: 

Long-Term Conditions Group

  • Meet others living with long-term conditions. 
  • When: First Thursday of every month, 10:30am to 12:30pm 
  • Where: Boardroom, Frome Medical Practice 

COPD Support Group

  • For people with lung conditions such as chronic bronchitis and emphysema. 
  • When: Second Thursday of every month, 10:30am to 12pm 
  • Where: Education Room, Frome Medical Practice 

Pain Café 

  • For anyone living with chronic pain. 
  • When: Last Friday of every month, 11:30am to 1pm 
  • Where: Education Room, Frome Medical Practice

Drop-in support around Frome: Talking Café and Talking Bench

Talking Cafés are friendly spaces where you can meet new people, make friends, and chat with a Health Connector. We can tell you about local activities, support groups, and services that could help you. 

Talking Benches give you the chance to meet face-to-face outdoors. Look for the Talking Bench flag and come and say hello. It’s a great way to find out what support is available in the community and connect with others.

  • Talking Cafe: Monday 10am to 11.30am Cheese and Grain, Justice Lane, Frome BA11 1BE
  • Talking Cafe: Thursday 1pm – 2.30pm, Coffee#1, 15-16 Westway Centre, Frome BA11 1BS
  • Talking Bench: Every Friday, 11:30am to 12:00pm, outside Frome Library.

Get involved in our community work

There are lots of ways you can support health and wellbeing in your local area. 

Train as a Community Connector 

Community Connectors are local volunteers who help friends, family, and neighbours find support.

They share information about groups, services, and activities that can improve health and wellbeing.

Training is free and open to anyone who wants to make a difference. 

Join our Postcard Project 

Health Connections runs a postcard project in Frome and the surrounding area.

Anyone can take part. If you’d like to receive a free, regular postcard from one of our volunteer writers, get in touch. It’s a simple way to feel connected and brighten someone’s day.
