Led by Julie, the Health Connections Team offers free support to help people improve their health and wellbeing.

They provide advice, signposting and practical help to connect patients with local services and community groups.

#hellomynameis is a campaign to encourage and remind healthcare staff about the importance of introductions in healthcare, helping us to provide truly person-centred, compassionate care.
