Live Well - Supporting Your Health in 2025

 

As we start a new year Frome Medical Practice is working with a range of providers as well as expanding services at the practice, so patients can access a range of free resources to support their health and wellbeing. 

The New Year can be a time for resolution, of setting new goals and intentions. The practice wants to support patients and help them achieve any health and wellbeing goals they may have now and throughout 2025.  

This is happening in a number of ways. From the introduction of new health apps for patients to use, to group support sessions, to a comprehensive online directory, the practice hopes that if you need help to live well you can find it.

Dr Helen Kingston, a GP at Frome Medical Practice, said, “We know that small changes can lead to big differences in how we all feel, both physically and mentally. What we are sharing here are a number of different resources patients can use to help themselves. Some of these can be used at home, others might involve coming into the practice. Either way, we know that all of these resources can help you take those small steps that can help improve your health and wellbeing. I’d really encourage you to take a look at what’s available and give some of them a try.”

 

Thymia - supporting your mental wellness

Our patients have free use of the thymia mental health wellness check. In the same way that a blood pressure cuff or thermometer tracks your physical health, thymia does the same for your mental health.

Thymia has been designed to help people better understand their mental wellbeing.  It can also spot changes in our mental health over time. This means our patients can seek support if they notice any changes in their mental wellness.  Any of our patients aged 18 and over can use thymia.

lumin&us App - free children’s mental health support 

We are offering parents and carers of children aged 3 to 12 free use of a new, neuroscience and play based app called lumin&us®.

Habits For Health - helping you live well  

Habits For Health is our interactive healthy lifestyle programme, which helps you identify what’s important for your health and supports you to make small changes in your life to help you achieve bigger health goals that will improve your overall health and wellbeing. These sessions are led by one of our Health Connectors and take place at the practice.

Habits for Health covers the following topics:

  • Setting goals for your health
  • Getting better sleep
  • Moving more
  • Taking care of your emotional wellbeing
  • Eating healthily
  • Connecting with others

Support for young people 

Our Young Person’s Clinic is available for patients aged 13 to 19 years old and offers support for sexual health and contraception, anxiety, stress and mental health. It takes place on a Wednesday afternoon, from 3pm to 5pm. No appointments are necessary. Just turn up at the practice, go to Area 7 (upstairs) and someone will book you in to chat with a member of the team. 

Young Somerset will be joining us at the Young Person’s Clinic in the new year to offer mental health support. Watch our social media for updates about this.

Health Connections Frome - our social prescribing team 

Our Health Connections Team is available to support our patients with their health and wellbeing, helping them to live well, in a number of different ways.

Only Order What You Need 

We need your help to reduce the amount of medicines that are wasted unnecessarily. This will help you live well and will also help the planet. 

Stay Safe 

  • Stockpiles of medicines kept at home can be dangerous for children and pets. 
  • Medicines are only prescribed for you and for safety’s sake should not be shared with anyone else. 

Stay Well

  • Unused medicines expire and might not work as intended, meaning your care might be compromised. 
  • Stockpiling medicines worsens drug shortages.

Stay Green 

  • Only ordering what you need means you are helping the environment. Once medicines have left the pharmacy they cannot be recycled or used by anyone else. 
  • When you dispose of medicines properly, by handing them into your pharmacy, you are helping to ensure waste medicines do not pollute our rivers and seas. 

How can you help? 

  • Check what medicines you have at home before you order your repeat prescriptions. If you have enough, only request the medicines you need this time. You will still be able to order others again in future.
  • Contact our pharmacy team if you have any questions about how to change your prescription request. You can do this using our online form.

Digital Health Support

We offer digital health support to all our patients, to help them get online. Being connected online reduces barriers to support and helps people access services from their own home. Our Digital Health Team helps patients with downloading and using health apps, including the NHS App, and accessing our services online.

Digital health support is available at the following times and venues:

  • Tuesday 10am to midday in the Live Well room (main reception area) at the practice
  • Thursday 4pm to 8pm in the Live Well room (main reception area) at the practice

Digital Health Pop Ups

  • 1st and 2nd Tuesday of the month in Frome Library from 1pm to 3pm
  • 2nd Saturday of the month venue and time to be confirmed (please check our events page or social media for details)
  • 3rd and 4th Tuesday of the month Sainsbury’s, Frome from 1pm to 3pm
  • 3rd Wednesday of the month Fair Frome, Marston Trading Estate from 10am to midday

Digital Cafe

  • Every Thursday 10am to 11:30am, in the cafe at the Cheese and Grain
