Community Care Team
What does Frome Medical Practice offer?
Our Community Care team is there to support to people with comlex care needs in their homes and communities.
The team provide support to help people live as independently as possible. We assign patients to the Community Care team where appropriate.
Part of the role of the team is to follow up patients when sent home from hospital. These may be patients who have long term health care needs and/or need support once home.
The team work with other health and social care providers such as:
- District Nurses
- Older People's Mental Health
- Adult Social Services
- Occupational Service
- Physiotherapists
- Our practice's Health Connectors
Our Community Care Team phone line is open 10am to 3pm Monday to Friday.
