Community Care Team

Our Community Care Team supports people who live in care homes or are housebound. We also check in with patients after they leave hospital to make sure they have the help they need.

The team works with other health and social care services, including:

• District Nurses
• Dorothy House
• Older People’s Mental Health
• Adult Social Services
• Occupational Therapy

Together, they make sure your care is joined up and meets your needs.

If you are a carer, we can also offer support to you.
