Overview
You are invited to join our dynamic, innovative and welcoming practice providing primary care at Frome Medical Practice. This is an exciting opportunity at any stage of your medical career, which also includes opportunities for partnership in the near future.
We have a supportive and collaborative team of over 20 GPs and 150 staff. We are a large and modern, purpose built practice with a patient population of just under 30,000, who live in the attractive market town of Frome and its surrounding villages. We operate as a single site Primary Care Network.
The large number of GPs at the practice creates a collaborative and friendly atmosphere, and spending time together in daily coffee breaks, discussing cases in team meetings and socialising is highly encouraged and ingrained in our work culture. This is supported by a dedicated complex care team which manages all home visits and hospital discharges, as well as nurse practitioners, a mental health nurse practitioner, a first contact physiotherapist, and a large dedicated pharmacy team.
We are proud to have received National Awards for Sustainability and our Green Impact work and we have also been nationally recognised for our outstanding Health Connections (social prescribing) programme, which is based within the practice. As a high-performing and successful practice we have been accepted to join the new PCN Pilot Programme working with NHSE which will also provide exciting opportunities to shape and develop clinical pathways.
Externally, we have strong relationships with our local community and local healthcare providers and we are situated next to the local community hospital, where we collaborate in overseeing our community ward, as well as providing sessions for other local services such as the fracture clinic. We are co-located in our Frome Medical Centre with adult mental health, district nurses, an independent pharmacy and a cafe. This all supports our active approach to multidisciplinary working.
We are a recognised Learning Organisation with a strong focus on GP Education and development. We are also a GP Training practice, where many of our registrars choose to stay on with us and benefit from our mentoring scheme.

Job Purpose
During your defined weekly hours you will work as part of a highly professional team providing medical services to our patients.
Main Duties
- Responding to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral.
- Working closely with our care Navigation team to triage patients using our current chosen supplier “Klinik”.
- Providing appropriate preventative health care and advice.
- Adhering to the practice prescribing formulary.
- Preparing medical reports for insurance companies, employers and statutory bodies.
- Attending weekly team meetings and education meetings held on days when you are working.
- Contributing to the development of and adhering to protocols / systems for the management of common medical conditions.
- Participating in clinical governance activity and contributing to the improvement in quality of health outcomes through the practice’s audit programme.
- Clinically related administrative and non-clinical duties necessary for the delivery of GM.
- Providing other services outside of GMS as specified in contracts held by the Practice.
- Engaging in appropriate educational activity in order to maintain clinical competence and performance, including the completion of objectives agreed by yourself and the practice in an annual Personal Development Plan.
- Participating in the training and development of nurses, medical students and GP registrars in the practice.
- Supporting the practice staff and responding to requests for advice and assistance from the practice care navigation, care coordination and nursing staff.
- Fully participating in extended hours offered by the Practice.
The above responsibilities will be undertaken upholding the following Best Practice Guidelines that all professionals working in The Frome Medical Practice have agreed to adhere to:-
- To be available to take over the responsibility of patient care from the Out of Hours service at 8am on a working day.
- To continue to work until all clinical tasks have been completed. Where this involves standing by ready to be called, to be available until the end of the session. To make arrangements for others to cover when planning to leave before the end of the session.
- To maintain the Practice’s high clinical standards by using clinical governance audits and other information to review patients results, prescribing, disease management etc.
- To undertake daily tasks such as results, letters, reports, repeat prescribing etc. for one’s own list and for other teams as required.
- To maintain the financial security of the organisation by ensuring accurate and efficient responses to income generation such as reports, dispensed prescriptions, private forms etc.
- To undertake, as part of a team, those duties specified as being required.
- To work as a team to cover the request for out of surgery contacts.
- To participate in the educational and organisational activities of the practice in a way that maintains high standards and encourages the development of The Frome Medical Practice as an approved “Learning Organisation”.
- At all times to behave in a professional way that encourages quality practice and the development of team spirit.
- Mandatory completion of Type 2 Self-Assessment form (annually in Feburary)
