Overview

The Health Connector will work in delivering person-centered care and support as part of the Health Connections team. Health Connectors support people to better self-manage their long-term health conditions / health and wellbeing in the community through a combination of individual and group work.

The working pattern for this role is Monday 9am until 5pm (30 min lunch), Tuesday 9am until 5pm (30 min lunch), Thursday 12pm until 8pm (30 min lunch). However, full time hours could be considered.  Evenings & Saturday hours must be worked around once per month and hours will be given as TOIL the following week. This is for a Fixed Term Contract to 30 June 2027 with the possibility of extension subject to contract.  

The salary for this role is currently £26,886.23 per annum pro rata and will increase to £30,229.90 per annum pro rata from 1 April 2026.

 

Responsibilities will include

  • Helping people to improve their health, wellbeing and social welfare by connecting them to community services. By increasing people’s active involvement with local diverse communities, personal and community resilience is strengthened - and health and wellbeing inequalities can be reduced.
  • Deliver Health Connections Mendip, projects within the community, including Talking Cafes and the Healthy Lifestyles programme, face-to-face or online as appropriate.
  • Identify gaps in service provision through speaking to patients and feed back to the Senior Health Connector / Area Lead.
  • Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what matters to them, with the aim of: improving people’s levels of ‘activation’, empowering people to manage their own health and improve their health outcomes.
  • Manage and prioritise a caseload of patients. Understand when it is appropriate or necessary to refer people back to, or on to, other health professionals/agencies.
  • Support the promotion of the Health Connections Mendip directory/website, resources, training and one-to-one support across the locality.
  • Attend and contribute to team, practice and Primary Care Network meetings and events as required by the service.
  • Participate in regular supervision, reflective practice and continual learning

Essential qualifications and experience:

Qualification

Desireable

  • Degree or other qualification in relevant area (or equivalent work experience)
  • Training in health / motivational coaching and interviewing or equivalent experience.

Experience

Desireable

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
  • Experience of working with people one-to-one using behaviour change, motivational interviewing and goal setting tools
  • Experience running and facilitating groups.
  • Experience of working with the Voluntary, Community and Social Enterprise (VCSE) sector (in a paid or unpaid capacity)

Skills and knowledge

Essential

  • Knowledge of the personalised care approach.
  • Excellent communication skills both verbal and non-verbal, 1:1 and with groups, and good written skills.
  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers.
  • Understanding of the needs of people with long term health conditions.
  • Ability to organise, plan and prioritise on own initiative, including when under pressure, to meet deadlines and achieve targets.
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.

Desireable

  • Local knowledge of VCSE and community services in your area.
  • Knowledge of how the NHS works (including primary care), local/national health policy and public health initiatives.

  • Willingness to work flexible hours when required to meet work demands.
  • Full UK Driving Licence
