Overview
You are invited to join our dynamic, innovative and welcoming practice providing primary care, at Frome Medical Practice.
We have a supportive and collaborative team of over 20 GPs and 150 staff. We are a large and modern, purpose built practice with a patient population of just under 30,000 and operate as a single site Primary Care Network.

The hours for this role are 37.5
- Monday: 9am to 1pm & 1:30pm to 5pm
- Tuesday: 8am to 12:30pm & 1pm to 5pm
- Wednesday: 8am to 12:30pm (Half Day)
- Thursday: 8am to 12:30pm & 1pm to 4pm
- Friday: 8am to 12:30pm & 1pm to 5pm
Job purpose
To play an active role in providing a high level of administrative support to clinical teams with a specific focus on complex care to maintain excellent levels of patient care and support.
Main responsibilities
- Secretarial and administrative support for Clinical staff to support patients.
- Supporting patients both over the phone and face to face and liaising with clinicians as required.
- Feeding back of blood/radiology/urine results to patients
- Managing E Referral lists, ensuring correct pathways are followed and liaising with Clinicians as required.
- Supporting both GPs and NPs on Complex care team. This includes liaising with Secondary Care and wider services on behalf of the GP/NP
- Managing the completion of all paperwork and liaising with family members following the death of patients.
- Arranging FIT Notes for patients.
- Managing and taking appropriate action of emails received from Secondary Care and third parties.
- Managing the completion of Safeguarding GP/NP report requests.
- Participate in the induction, education and training of all members of staff from all disciplines.
- Supporting integration & knowledge sharing across the practice.
- To actively engage with patients with dementia and cancer diagnosis and their carers to provide support linking to other support agencies and in the booking of appointments for patients.
- Supporting the organisation of vaccination programmes for Housebound and Care Home Patients
- Support admin related tasks related to the Health Connections Team
- Supporting other administrative and secretarial duties as required by the practice.
- Supporting Quality Outcome Framework (QOF) work including health reviews for Learning Disabilities, Dementia and Mental Health
- Flexible working is required to cover evening and occasional weekend shifts if required due to holidays and sickness or to support Flu vaccination clinics and other practice meetings such as the Patient Participation Group.
