Overview
You are invited to join our dynamic, innovative and welcoming practice providing primary care, at Frome Medical Practice.
We are looking for an organised and proactive administrator to support our clinical teams with long-term condition management, screening programmes and other patient care initiatives. A significant part of the role involves communicating with patients by telephone, text message, letter and email to arrange reviews, appointments and follow-up care. You will play an important role in supporting Quality and Outcomes Framework (QOF) activity, helping patients access the right care at the right time and contributing to improved health outcomes across our practice population.
We have a supportive and collaborative team of over 20 GPs and 150 staff. We are a large and modern, purpose built practice with a patient population of just under 30,000 and operate as a single site Primary Care Network.

We are looking for someone to work 22.5 to 30 hours per week. Our preference is for the hours to be worked across Tuesday to Friday, however we can offer flexibility around working patterns, including start and finish times, for the right candidate.
Job purpose
To play an active role in providing a high level of administrative support to clinical teams with a specific focus on complex care to maintain excellent levels of patient care and support.
Main responsibilities
- To monitor our chronic disease patient lists, ensuring they receive annual reviews and ongoing follow up where control is suboptimal.
- Be aware of and promote general health opportunities for all patients to improve population understanding of healthy active lives, good diets, and weight management in prevention of metabolic syndrome and diabetes.
- Recording immunisations given in school on EMIS, when lists are received from child health.
- To support in the cervical screening recall, engaging with patients, booking appointments and correct coding on EMIS.
- Processing and filing cervical screening results and adding diary dates.
- Monthly audits for pessary ring and depo telephoning patients, booking appointments, sending SMS/ letters. Ensuring diary dates are up to date.
- BP monitoring admin. Managing tasks list and patient lists from searches. Recording BP’s on EMIS and actioning results. Organising loaning of BP monitors to patients.
- Actioning contact forms received for INR, alcohol audits and general queries relating to workload.
- Running and actioning weekly searches. Adding correct codes and sending invites where appropriate or sending tasks to clinicians. Using choose & book system to action referrals.
- Coding of breast screening attendance, when lists are received from Avon Breast Screening.
- To support in respiratory assessment/ spirometry appointment booking. Actioning tasks received, updating spreadsheet and inviting patients in for appointments.
- Generally supporting patient wellbeing by communicating with patients using various methods - telephone calls, text messages, letters and emails.

Qualifications & Training
Essential
- Good Standard of education to include GCSE Grade C/4 or above (or equivalent) in English Language and Mathematics
Experience
Essential
- Experience of working in an administrative and/or customer service environment, including communicating with the public by telephone, email and in writing.
- Experience of using computer-based information systems and maintaining accurate records.
- Experience of managing and prioritising a varied workload to meet deadlines.
Desirable
- Experience of working within Primary Care, the NHS or a healthcare setting, including use of clinical systems, medical terminology and patient recall or screening processes.
