Care Navigation Team

Led by Hannah, the Care Navigation Team makes sure patients receive the right care, at the right time, with the right professional.

They:

• Help patients book appointments with the most appropriate clinician
• Guide people to local services and community support
• Answer queries and provide information about practice services
• Maintain strict confidentiality at all times
• Support patients throughout their care journey

Their work helps patients access the right help quickly and ensures the practice runs smoothly.
