Care Coordination Team

Led by Sarah, the Care Coordination Team helps patients get the right support and assists our GPs with essential tasks.

They:

• Arrange appointments and referrals
• Help patients understand and follow their care plans
• Connect people with community and social services
• Act as a point of contact for ongoing support
• Manage referrals, results and inbound documents for GPs

Their work ensures patients have access to everything they need and gives GPs more time to focus on direct patient care.
