Feedback opportunity for patients with learning disabilities

We would like your help to make our health checks better.

We would like you to give us feedback about your experience when you visit us for your annual health check. We want to know what we are doing well and what we could do better. Your answers will help us improve visits to the practice for everyone.

There are 3 ways to give feedback:

1. Give feedback online
2. Ask reception for a paper feedback form
3. Give someone your feedback at the Hub drop in. This takes place in the Live Well Room, Frome Medical Practice every Wednesday from 10am to midday.

Your friends, family and carers can give feedback too.
