Frome Medical Practice

Frome Medical Practice delivers primary care services to nearly 30,000 patients from the attractive market town of Frome and surrounding villages.

Our last CQC inspection was rated outstanding and we are Somerset Practice of the Year 2017. We are well known for our innovative Health Connections project and our Care Coordination Hub. Both of these are underpinned by the strong relationships we have with established with the local community and other serice providers to really support and improve patient care. 

For the last two years we have also been recognised by the Royal College of General Practice as leading the way in sustainability and green impact

Why Frome?

Frome has been highlighted as one of the 'best places to live in Britain' and is a welcoming and vibrant place to live, visit and work. Whether you enjoy nature, culture and arts, shopping or history - Frome has plenty to offer.

Located on the edge of the beautiful Mendip Hills, in the East of Somerset, the town is home to just over 27,000 people, with many more living in the numerous surrounding villages. The Roman city of Bath is about 13 miles away. 

The town of Frome is governed in a three tier local authority arrangement - Frome Town Council, Mendip District Council and Somerset County Council. It is the largest town in Mendip and the fourth largest town in Somerset, yet it retains a very independent nature - partly due to it's geographical location some distance from the administrative centre of the county but also because, since 2015, the town council has  been made up of all independent councillors. 

The Frome Town Profile presents data relating to the local population on a range of issues that can influence health, including deprivation, housing, education, employment, crime and hospital admissions. 

Health Connections Mendip

Also known as the Frome Enhanced Model of Primary Care or 'Compassionate Frome', Health Connections Mendip provides an innovative approach to health care - linking routine health care with social connectedness; looking at people's community networks and linking people to their own existing support networks, as well as the extensive range of community activity and groups that exist in and around Frome. 

This way of working has been shown to improve patient outcomes and improve the working lives of clinicians. And it is reducing emergency admissions to hospital, at a time when emergency admissions across Somerset have been rising.
