Habits for Health programme launched

 

Patients invited to improve their health and their happiness

Frome Medical Practice is inviting patients to take part in a programme designed to help them ‘be healthy, be happy’.

The programme, which takes place for one hour a week over five weeks, is aimed at patients with one or more long term conditions and aims to help them improve their health by making small changes, which in turn can improve their physical and mental wellbeing.

Dr Rob Taylor said, “There’s growing evidence that many of our long term conditions can be helped by the way we live our lives. Chronic health problems are not well managed by medicine alone and so we want to offer patients another way to help improve their health and overall wellbeing.”

Jo Fordham, Senior Health Connector in the Frome Health Connections team helps to deliver the programme. Jo said, “The programme has really helped patients in the past and we’d encourage patients to sign up for our next sessions. We start by looking at your personal needs and goals and help you work out what you can change. These are often small changes that you can stick with, which can lead to a real improvement in your health and happiness.”

The programme looks at various aspects of patient’s lives, including movement, sleep, eating and emotional health and social wellbeing, which all impact on people’s health and how they feel and supports patients to put new habits in place.
