Live Well Sessions 

Our Lifestyle Care Coordinator is available to support patients at our Live Well sessions. Find out more about what these sessions are for and how you can use them below.

When and where do Live Well sessions take place? 

The Live Well sessions take place at the practice several times a week. Sessions take place in the Live Well space (glass room on the left hand side as you enter the main reception area at the practice) as follows: 

  • Tuesday 12.00pm - 2.00pm 
  • Thursday 10.00am - 12.00pm 

You don’t need to make an appointment to use the Live Well sessions. Simply drop in and we will see you. If there is another patient already seeing our Lifestyle Care Coordinator, please just wait and we will see you as soon as possible. 

 

What can I use the Live Well sessions for? 

Live Well sessions aim to support you to learn more about what you can do for yourself to live well. Very often patients want to make small changes that will help improve their health and wellbeing but they don’t necessarily know where to start. These sessions are there to help you with this. 

We can do some practical things at the sessions, like help you with blood pressure readings and checking your weight and / or height. 

We also encourage patients to complete a quick lifestyle questionnaire at the sessions. We can then chat about your answers, and we can let you know about support that’s available at the practice and in the community, if it’s needed, including referring you to Health Connections Frome or signposting you to other services that you might find helpful. 

If you have any questions about your health and wellbeing that aren’t medical we can help you with finding answers or information that can help you. Some examples of what we can help with include information on support available for particular health conditions, finding out what opportunities there are to be more active, how you can get support to stop smoking, other ways to improve your health and wellbeing and so on. 

We will run focus weeks around certain awareness campaigns. So, for example, we will focus on a campaign like the ‘Know Your Numbers’ blood pressure awareness week in September and encourage more patients to come forward to get their blood pressure checked.

Who are the Live Well sessions for? 

The sessions are for any patients registered at Frome Medical Practice who want to find out more about the ways they can support their own health and wellbeing.
