What does Frome Medical Practice offer?

Frome Medical Practice has a dedicated team who help to support patients with Diabetes.

Patients with Diabetes can access the following support and information.


Annual Review

Patients receive a full annual review.

You will first be invited to come into the practice and see a Health Care Assistant face to face. The Health Care Assistant will check the health of your feet and will then take a blood sample and a blood pressure reading.

The Health Care Assistant will then book you a face to face appointment with a Practice Nurse in approximately two weeks time, to review your results and agree your treatment plan. 


NHS Diabetes Prevention Programme

The NHS Diabetes Prevention Programme will help you take control of your health, supporting you to make changes to your diet, weight and the amount of physical activity you do. If you have been told you are pre-diabetic and / or have received an invite letter, please self-refer to this programme.

The NHS Diabetes Prevention Programme is available online and in person. Group sessions are hosted in Frome and the surrounding areas. Details of current groups that are running locally can be found on the NHS Diabetes Prevention Programme website.

MYWAY Diabetes - Online Platform

Patients from Frome Medical Practice who have diabetes are encouraged to use the MYWAY Diabetes online platform, which aims to support people with diabetes, as well as their family and friends.

Diabetic Eye Screening

In addition to the annual check within the Practice, patients with diabetes will be invited to an annual Diabetic Eye Screening appointment.

If you have Diabetes and are aged 12 or over you’ll receive a letter every year inviting you to this appointment.

This programme is coordinated by Somerset Partnership NHS Trust. Appointments are generally held at Frome Community Hospital for this service.

Education Session Recording

In our Education Session recording, Dr Rob Taylor and Jo Fordham, from the Health Connections Team, share some simple diet principles to help patients manage Type 2 Diabetes. The aim of these principles is to lose weight, reverse Diabetes and reduce medication use.

The session is aimed at anyone with Type 2 Diabetes or Prediabetes, those with an interest in Type 2 Diabetes or people who are a carer of someone with the condition.

Prediabetes is when someone has a blood sugar level above the normal range but not high enough to be diagnosed as having Diabetes. If your blood sugar level is above the normal range then your risk of developing full blown Diabetes is increased. We encourage you to watch the recording of the education session if we find that you have Prediabetes.

The session covers the following: 

  • We will look at how diabetes affects your body. 
  • We will learn how lifestyle changes can help you manage your condition.
  • We will cover various lifestyle choices, including sharing information with you on diet and exercise.
  • We will also consider how we manage our sleep, stress and general health and well being.
