Patient Services Manager

Monday to Friday: 37.5 hours/week
Annual salary: £26,500 to £29,500 (depending on experience)

We are looking for an experienced Manager to lead our Care Navigator and Scheduling Team. This newly created role will work as a key member of our Operational Delivery Team (ODT) and will have a strong focus on optimising the service provided by our call centre and front of house teams. Previous experience of working within the NHS is not essential but an ability to ensure the delivery of key deliverables ensuring the best possible patient care and experience of working in a fast-paced work environment along with a passion for making a difference are highly desirable.

You will have:

  • Previous Call Centre Management experience and/or Supervisory Management skills managing multiple teams.
  • The ability to listen effectively to both staff and patient feedback and the ability to analyse and solve problems logically and efficiently.
  • Experience of managing and analysing data on a daily/weekly basis including the effective monitoring of key performance indicators.
  • Management of patient/customer feedback.
  • Experience of Health & Safety, e.g. risk assessments.
  • Very strong written and verbal communication skills.

Responsibilities will include:

  • Ensuring the effective running of the call centre and patient meet and greet service.
  • Working collaboratively with the ODT to ensure the achievement of goals and objectives and ensure the growth and strategic development of the practice.
  • Staff Management of multiple teams, to include recruitment, training and development and on-going performance management and staff development.

Clinical Pharmacist

Monday to Friday: 37.5 hours/week
Annual salary: £50,500 to £54,500 (depending on experience)

We are seeking an experienced Pharmacist to fill a key role in our newly formed Medicines Management Team. As a Primary Care Network in its own right and with nearly 30,000 patients, the Clinical Pharmacist will be responsible for medicine management optimisation and safety within the practice. There will also be a strong focus on multi-disciplinary working across the practice.

You will have:

  • Registration with the GPhC along with Membership of the Royal Pharmaceutical Society.
  • A Masters Degree in Pharmacy and be an Independent Prescriber.
  • With at least 2 years of post qualified experience, you will also have experience of managing, advising and reviewing general medicine optimisation issues in core areas for long term conditions.
  • Experience of planning, managing and monitoring a prescribing team.
  • Experience of medication reviews.
  • Provide evidence of experience of Quality Improvement processes.
  • Very strong written and verbal communication skills and an ability to work effectively under pressure.

Responsibilities will include:

  • Play a lead role in our Medicines Management Team to optimise prescribing safety and take part in Quality Improvement.
  • Face to face and telephone consultations to support chronic disease.
  • Medication reviews including reconciling medicines following hospital
    discharge.

Salaried General Practitioner – Frailty Lead

Monday to Friday: 4 – 8 sessions per week

We are currently seeking an experienced GP to take a lead role in our Practice with regards to Frailty Care. Working as a key member of the Practice team, this role will include working on assessment beds at our local community hospital based next to the Practice and overseeing a number of Nursing homes with the support of our visiting nurse team.

The main roles and responsibilities will involve the following key areas of activity: -

  • Clinical – to undertake regular ward sessions and duty GP clinics. Lead the management of frailty as a long term condition and implement GMS frailty requirements. Optimise practice QOF compliance and ensure the delivery of a holistic approach to frailty care.
  • Service development – Develop protocols and systems to manage common medical conditions, participate in clinical governance activity and act as the link with local geriatric services.
  • Administration – preparing medical reports along with providing other services outside of PMS as specified in contracts held by the practice.
  • Education and team integration – Provide mentoring and clinical supervision to other clinical staff, engage in appropriate educational activity to ensure clinical competence and performance.
  • Organisation ethos – Understand and respond to the particular needs of older people with frailty and co-existent mental health problems in addition to encouraging patient empowerment and resilience for self-care through recognition of networks of support.

Advanced Nurse Practitioner in Geriatrics

Monday to Friday: 37.5 hours per week.

We are currently seeking an experienced Advanced Nurse Practitioner to work as part of multidisciplinary team. Ensuring appropriate in-patient care and assessments through ward rounds in both a community ward setting at our local community hospital based next to the Practice, and overseeing a number of Nursing homes with support of our visiting nurse team, this role will proactively work to ensure patient centred care, support of carers and efficient care co-ordination for safe and effective care of older people.

The main roles and responsibilities will involve the following key areas of activity: -

  • Assess, plan, implement and evaluate specialist treatment and care to older people promoting independence and autonomy. Understand and respond to the particular needs of older people with frailty and co-existent mental health problems and engage with Practice QOF compliance for vulnerable groups.
  • Provide holistic care to support people using a multi-professional and interagency approach ensuring that care planning and decision making is co-ordinated and communicated across all involved organisations.
  • Engage in appropriate educational activity in order to maintain clinical competence and performance and participate in the training and development of nurses, medical students and GP registrars in the Practice.
