We are seeking a Clinical Pharmacist to join our Medicines Management team. You will play a key role in the team, ensuring medicines management optimisation and safety. The role will include multidisciplinary working with other clinical teams.

This is an exciting opportunity to join a dynamic, innovative, welcoming and supportive team providing primary care, at Frome Medical Practice. We are a single site practice and Primary Care Network in our own right with a patient population of just under 30,000. We were Somerset CCG Practice of the Year 2017.  We have received National Awards for Sustainability and our Green Impact work and have also been nationally recognised for our outstanding Health Connections (social prescribing) programme, which is based in the practice.   We have strong relationships with our local community and local healthcare providers and are situated next to the local community hospital.


Essential qualifications and experience:

  • Registration with the GPhC along with Membership of the Royal Pharmaceutical Society.
  • A Degree in Pharmacy and be a non medical Prescriber, or working towards this.
  • Minimum 2 years of post qualified experience
  • You will also have experience of managing, advising and reviewing general medicine optimisation issues in core areas for long term conditions.
  • Experience of medication reviews.
  • Excellent communication skills and an ability to work effectively under pressure.

Responsibilities will include:

  • Playing a key role in our Medicines Management Team to ensure medicines management optimisation and safety. It will include multidisciplinary working with other clinical teams.
  • Medication reviews - ordering relevant monitoring tests and oversight of reducing polypharmacy. 
  • Review and action any necessary changes to repeat prescribing and medicines management policies 
  • Manage repeat prescribing re-authorisation process 
  • Reconcile medicines on discharge from hospital 
  • Undertake clinical medication reviews
