Care Coordination Hub
The Care Co-ordinating Hub at Frome Medical Practice offers support to patients in their communities to help manage their health care needs and ensure people are able to access the support they require to live as independently as possible.
Part of the role of the Hub is to follow up patients on discharge from hospital who may have long term health care needs and may require support once home.
Working closely with other health and social care providers such as District Nurses, Older People's Mental Health, Adult Social Services, Occupational Service, Physiotherapist and Mendip Health Connections we can co-ordinate care and the support required for our patients and their families thereby helping them to better manage their conditions and live more independent and empowered.
Our carers are also supported as part of the Hub.
#hellomynameis is a campaign to encourage and remind healthcare staff about the importance of introductions in healthcare, helping us to provide truly person-centred, compassionate care.
