Health Connector (fixed term)

Join the Health Connections Mendip team

● Salary: £17,931.00
● 37.5 hours a week
● Fixed term – one year from start date
● To cover Glastonbury, Street and Wells area.

Are you good with people, a listener and motivator, flexible and adaptable with an understanding of voluntary and statutory services across Mendip?

We are looking for a compassionate, self-motivated and organised person who thrives on supporting others 1:1 to join an innovative primary care project in Mendip. The person will work in a person centred way to help local patients to manage their health and wellbeing, make lifestyle changes, access health and community services and develop their support network. The role will involve working in doctor’s surgeries, people’s homes and in a range of other health and community settings. Health Connections Mendip works within a multi-disciplinary team ranging from the voluntary sector to palliative care through to secondary care.

Monday to Friday: 20 hours per week
Annual Salary: £15,310.00 (pro rata under review)

We are currently looking for a hardworking, reliable person with excellent attention to detail skills to join our innovative forward thinking team at Frome Medical Centre.

Essential Qualifications:

  • Minimum 3 Grade C and above GCSE / O level passes, to include English Language and Maths.
  • Experience using Word and Office as well as other computer based information systems
  • Experience communicating with a range of people: face to face and on the telephone.
  • Enthusiastic team player
  • Ability to work well in a busy environment

Responsibilities will include:

  • Responsive to the needs of patients by phone and face to face
  • Accept and deal with all requests from clinical staff in order to aid the smooth running of the Practice
  • Manage the visits line and organisation of visits and contingencies Be responsive to and act upon feedback provided during coaching, call listening/recording sessions and developmental feedback
  • Ensure all staff and patient areas are kept tidy and take any necessary action to problems occurring
