Make Your Choice - Choose if data from your health records is shared for research and planning  

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning, as the video below explains:  

You can choose to stop your confidential patient information being used for research and planning.  You can also make a choice about this for someone else, like your children under the age of 13.  

 

Making Your Choice 

If you do not want your identifiable patient data to be shared outside your GP Practice for purposes except for your own care,  you can register an opt-out with us. This is known as Type 1 Opt Out. If you wish to do a Type 1 Opt Out you can download the form (click here), complete it and return it to us.   

The second type of opt out is the National Data Opt Out. This will prevent NHS Digital from sharing your anonymised data with third parties such as medical research or drug companies.  

If you have opted out with the practice using the Type 1 Opt Out form you do not need to complete the National Data Opt Out. If you complete the Type 1 Opt Out form after this deadline you may wish to complete the National Data Opt Out too. Click here to do this.
