Meet Rachel, our Digital Health Connector

It's increasingly important that patients can access their health information online. Many of our patients are already registered to use our online services and we are now encouraging them to also start using the NHS App.

We appreciate that some of our patients will need help with this.  Our Digital Health Connector is here to support people with downloading Apps, registering to use them and understanding how they can then access their health information online. 

Rachel started in her role as our Digital Health Connector in the Spring of 2020. Here she describes what her job is and how she can help you, in her own words. 


Meet our Digital Health Connector 

Hello, my name is Rachel and I'm the Digital Health Connector at Frome Medical Practice. My role is to help you get connected with our digital services, such as the NHS App

I've worked in the NHS for a number of years, training staff and patients in using IT software and I am passionate about helping others get the best out of the services that are available to them. 

I've set myself a personal challenge to reach out to as many of our patients as possible to support them in using the NHS App. 

This App allows patients to access a wide range of NHS services, including their medical records, test results and ordering repeat medication. It's a real game changer! 

I believe that everyone should have easy access to their own health information, so they can be more engaged in decisions about their care and support themselves better.

I'm here to make sure all our patients are supported, whatever their technical abilities. With patience and encouragement everyone can benefit from technology such as the NHS App, so no one misses out.

How To Access The NHS App 

If you have a smartphone or a tablet, you can now download the NHS App from your mobile device’s app store and follow the easy step-by-step registration instructions. It will help if you know your NHS number (though this isn’t essential), and you will need to have a form of ID (passport or driving licence) to hand.
