Register with Frome Medical Practice
To register as a patient of the Frome Medical Practice you will need to:
- Download and sign the Security Policy Information Form
- Download and complete the Frome Medical Registration Questionnaire
- Complete the downloadable Family Doctor Registration Form
When you have completed these 3 forms please bring them into one of our Surgeries. If you are currently taking repeat medication we will book an appointment with a GP for a review
Please ensure that you complete your 10 digit NHS number and sign at the bottom of the registration form
